Two methods of referral are available....

1.Call the Center for Pain Medicine and Regional Anesthesia at 319-356-2320 from 8:00 am - 5:00 pm Monday through Friday to make an appointment for your patient. You will be asked to send copies of the patient's medical records so that they may be reviewed prior to the appointment.

2. Fill the UIHC's Universal Referral Form and click submit.

To help expedite your request, please have the following information available:

• Caller's name and affiliation.
• Town/City.
• Department or physician with whom you want to speak.
• A brief description of the nature of your request.
• Patient's name and demographics. 

Following the appointment, the physician caring for your patient will send a letter back to you informing you of their diagnosis and treatment plan. Staff will strive to work closely with you to ensure the best possible care for your patient.

Consultations

If you require a consultation and have questions about the diagnosis or treatment options for a specific patient, call the Center for Pain Medicine from 8:00 am - 5:00 pm, Monday through Friday.
